Dollars and Sense

As the healthcare debate rambles on, we have been thinking about costs, ethics, and justice. How much should a birth cost? Midwives are well aware that not all births are equal, yet midwifery care within a setting (e.g., home, birth center, hospital) is generally a fixed fee. Should a midwife charge less for an "easy" birth and more for a "difficult" birth? What about charging extra for twins? We ask these questions with tongue in cheek, but there's a kernel of truth underneath: how can the midwife be adequately compensated for her work? Add regional differences in fees to the mix and the issue becomes even more complex. So, how much should a birth cost? We welcome your comments!
